Nottingham and Nottinghamshire ICB’s Personalised Care Programme is a success!
This two-year programme aimed to reduce surgery cancellations and improve patient outcomes.
Here’s what we achieved:

- Reduced cancellations: Our Link Workers helped patients overcome barriers to surgery, leading to more procedures happening on schedule.
